About how long will it take for me to receive my retirement plan distribution?

Depending on your plan, processing time is approximately 10-15 business days.

To Whom, Where & How should I send my completed physical forms to be processed?

If you are unable to process your distribution online through your plan’s website, please return your signed form to Plan Integrity Partners as follows:

Form Completion Tips

If your account has both Roth & Pre-tax funds, please be sure to provide an account number for both fund types on your distribution form. This does not apply if you are requesting a rollover to your new employer’s 401(k) plan.

If your form includes a Spousal Consent section (for Qualified Joint & Survivor Annuity – QJSA), this MUST be completed. The consent must be signed AND witnessed by a Notary Public or the Plan Sponsor. If you are not married, simply indicate so.
